The
purpose of this memorandum is to reconfirm the need for securing proper
documentation when an employee will be absent for an extended period of
illness. In order to protect both the
employee and the College, appropriate documentation from the employee's
physician should be secured from the employee as soon as possible after the
illness occurs, indicating date of commencement, general nature, and expected
period of the illness. Upon receipt of
such information, a confidential form should be prepared, putting the employee
on sick-leave status.
Before
an employee is allowed to return to work, appropriate documentation from the
employee's physician should be obtained, indicating employee is able to resume
his (her) full-time duties. When such
documentation is received, a confidential form should be prepared, indicating
date of employee's return.
Documentation
described above is necessary to implement applicable insurance coverage for the
employee and to protect both the employee and the College when he (she) is
ready to return to work.
